Migration from SBS-2008 to Server 2012 R2 and Exchange 2016 running on Hyper-V

I am going to be moving a customer from SBS-2008 running Exchange 2007. I have found information going from SBS-2008 to 2012 Essentials but not to Server 2012r2. I am sure there are steps missing or not accurate for migration.

 I am hoping you have some direct links to the steps needed to move from SBS-2008 [not SBS-2008r2]

My goal is to have 4 VMs running on Hyper-V.  They all will be on the base Server 2012R2. The first will be the  AD/DC and application server. The second will be Exchange 2016 (or if I cannot get there Exchange 2013).  The 3rd will be their Accounting Server, running Sage 300. Then finally the last server will be for running their CRM program, also Sage/Timberline Application.

I have been unable to find the specific document to walk me through this process. I have the new server sitting on a Lenovo RD550 rack mounted server with 2 Processors and 128 GB of RAM. I have the base server installed 4 times and all updates done on all 4. Now how much further can I go before taking the system on site to begin the migration process? 

Do I install the AD and setup the DC now or will that all occur during the migration steps? Do I use the same Domain name that is on the SBS-2008 server or start fresh?

Should I install Exchange 2016 or 2013, now or wait until the migration process request it be installed?
